our Vendor Routing Form.
Unscheduled shipments may not be accepted.
You will need a copy of the Leon Korol PO in order to complete this form.
If you do not have this, please e-mail us for the PO number and PO date.
Please complete one VRF for the total number of shipments by location.
If you have multiple FOB shipping points, please complete one VRF for the total by each FOB location.
